New Family Registration
Welcome and thank you for wanting to be a part of our community. It is important that ALL families are registered at St. Bernard. The parish that you register in does not have to be the parish that is assigned to your particular geographical region. Please see the attached document of the importance of registering at a Parish. /Registracion = Bienvenido y gracias por querer ser parte de nuestra comunidad . Es importante que todas las familias están registrados en San Bernardo . La parroquia que se registre en no tiene por qué ser la parroquia que se asigna a su región geográfica. Baptism
If you will be having your child’s Baptism at St. Bernard, please print and fill out form below, bring to the Parish office and schedule Baptism and Pre-Baptismal Class. / Bautismo = Si usted va a tener el bautismo de su hijo en San Bernardo , imprima y llene el formulario a continuación , llevar a la oficina parroquial y programar el bautismo y Clase Pre – Bautismal.
Pre-Baptismal Class for Guest
If you are a guest from another Parish and JUST NEED PRE-BAPTISMAL Class, fill out the form and send to the parish office. We offer Pre-Baptismal Classes on the 1st Tuesday of every month. Please let the office know you are coming. / Clase Pre- Bautismal de huéspedes = Si usted es un invitado de otra parroquia y sólo necesita la clase pre-bautismal, por favor de llenar el formulario adjunto y enviar a la oficina parroquial . Ofrecemos clases pre- bautismales los primeros martes del mes. Por favor, deje saber a la oficina la fecha que va a la clase.

Sacramental Request
If you need to request a Sacramental Record, please download and fill out the form below and return to the Parish Office. Only the person who received the sacrament or the parent or legal guardian of a minor child may request a sacramental certificate. Certificates are not issued to third parties. Requests are made either in person (after presenting a government-issued photo identification) or by a signed letter. Certificates are not issued for genealogical purposes. / Solo la persona que recibió el sacramento, los padres o el representante legal de un/a menor de edad podrá solicitar el certificado sacramental. Los certificados sacramentales no se entregan a terceros. La solicitud se hace en la parroquia ya sea en persona (después de presentar un documento de identidad del estado con foto) o a través de una carta firmada por el solicitante. Los certificados no se distribuyen para propósitos de investigación genealógica.
